REASON: Clarification of safety procedures and practices regarding access to the top of the tank

 

COMPLETION: WITH IMMEDIATE EFFECT

 

Special Safety Note:

 

NO pipe, tubing, or conduit on the tank should be used to tie off or used as anchor points as part of a fall arrest system.

 

Operator is responsible for all fall arrest devices and systems per operator’s company policies and procedures.
